What to Include in a Referral
- Patient name and contact information
- Reason for referral
- Relevant history or current diagnoses, if known
- Your name and contact information as the referring provider
- Any records you're able to share, with appropriate patient authorization

Communication Expectations
We're committed to clear communication back to referring providers throughout a shared patient's care. Any communication occurs only with the patient's authorization and in accordance with applicable privacy requirements, including HIPAA.
Referral Response Time: We strive to respond to referral requests during normal business hours.
