Paying Without Insurance

Self-Pay Options

Self-pay may be available if you do not have insurance, prefer not to use insurance, or your plan is not currently accepted.

Review current fees

Confirm current self-pay fees before scheduling your appointment.

Understand payment timing

Review when payment is collected and which payment methods are accepted.

Ask before your visit

Contact Prudent Haven if payment or coverage information is unclear.

What your cost may depend on

Your specific plan

Coverage varies between insurance products, employers and benefit designs.

Deductible status

Your responsibility may be higher when your deductible has not yet been met.

Copay or coinsurance

Your plan may assign a fixed copay or a percentage of the allowed amount.

Service and eligibility

Coverage may depend on the service, diagnosis, location or authorization requirements.

Important: Participation in an insurance network does not guarantee that every service will be covered. Benefit information available before a visit is an estimate and does not replace the insurer's final claim determination.

Before your appointment

01

Provide Accurate Insurance Details

Provide your current member and plan information during scheduling or whenever requested.

02

Review Available Estimates

Review any available eligibility and estimated patient-responsibility information before your appointment.

03

Confirm Plan Requirements

Ask your insurer about deductibles, copays, authorization requirements and telehealth benefits.

Self-Pay Patient Rights

Good Faith Estimates

If you do not have insurance or choose not to use insurance, you may have the right to receive a Good Faith Estimate of expected charges before receiving scheduled services. Ask the practice or applicable billing platform for information about obtaining an estimate.

A Good Faith Estimate is not a bill, and actual charges may differ if the scope of services changes. Learn more about these federal protections through CMS medical billing rights .
